• Step 2. Please fill out the Medical History form on our website or we can, e-mal, fax, or mail the form to you.

• Step 3. Once we have met with you with a face-to-face consultation to establish a patient-nurse practitioner relationship and reviewed your complete medical history form our nurse practitioner will select required blood/urine/saliva work evaluation. (If you already have blood work and it's less than a year old we can most likely use it.)

• Step 4. Once we receive your blood/urine/saliva work our Nurse Practitioner will review and help you design a program that ismost suitable to you.

• Step 5. Our Nurse Practitioner will contact you for a follow up visit consultation to discuss all dosages, side effects and results you should expect.

• Step 6. Start Feeling and Looking Great!!!!
